本研究旨在探討臨床護理師悲憫情感中的悲憫滿足、專業耗竭與次級創傷等三項情感與生理、心理、人際、專業與靈性等五類自我照顧狀態之間的可能關聯模式。本研究使用Stamm(2010)修訂之「專業人員生活品質」與張吟慈(2008)編製的「自我照顧量表」為工具,針對198位醫院臨床護理師進行問卷調查。本研究結果發現:(一)不管在專業耗竭、次級創傷或悲憫滿足上,都約有8成臨床護理師處於中等程度狀態,約有2成受試護理師有高程度的悲憫滿足,而幾乎沒有臨床護理師有高程度的專業耗竭和次級創傷。(二)臨床護理師在悲憫滿足程度顯著高於專業耗竭與次級創傷的程度。(三)臨床護理師在靈性與專業的自我照顧表現較佳,而在人際層面與生理層面的自我照顧偏弱,待加強。(四)未婚受試護理師比已婚受試護理師更容易產生專業耗竭,卻較不容易產生悲憫滿足感,而其他人口學與工作背景屬性相關變項似乎與護理師的悲憫情感高低無太大關係。(五)年齡較輕的臨床護理師在靈性層面的自我照顧較差;未婚護理師在生理、人際、專業與靈性層面的自我照顧較差;護理進階經驗資淺的護理師在專業與靈性層面的自我照顧較差;如果一周內,平均每天加班時數1小時以上的護理師則在生理層面的自我照顧較差。(六)悲憫滿足和所有自我照顧五個向度間皆呈現顯著的正相關;專業耗竭和所有自我照顧五個向度間皆呈現顯著負相關;次級創傷則只與生理自我照顧有顯著的負相關。(七)專業耗竭與次級創傷可以同時顯著預測生理自我照顧;悲憫滿足與專業耗竭可以同時顯著預測心理自我照顧與專業自我照顧;專業耗竭可以顯著預測人際自我照顧;悲憫滿足可以顯著預測靈性自我照顧。(八)靈性自我照顧程度可以顯著預測悲憫滿足感;生理與靈性自我照顧程度可以同時顯著專業耗竭感;生理自我照顧程度可以顯著預測次級創傷感。根據以上研究結果,研究者特針對臨床護理師、醫療機構及未來研究提出建議。
The purpose of this study was to investigate the relationships between compassion satisfaction, burnout, secondary traumatic stress, and five self-care domains including physiological, psychological, interpersonal, professional, spiritual among clinical nurses. This study used the "Professional Quality of Life" as amended by Stamm (2010) and the "Self-Care Scale" compiled by Zhang Yinci (2008) as tools to conduct a questionnaire survey of 198 hospital clinical nurses. The main results of this study showed that: (1) About 80% of clinical nurses had moderate states of burnout, secondary traumatic stress and compassion satisfaction, about 20% of clinical nurses had a high degree of compassion satisfaction, and barely any clinical nurses had a high degree of burnout and secondary traumatic stress. (2) The degree of satisfaction in compassion was significantly higher than that of burnout and secondary traumatic stress among clinical nurses. (3) Clinical nurses showed better in spiritual and professional self-cares, while their interpersonal and physiological self-cares were weak and needed to be strengthened. (4) Unmarried clinical nurses were more prone to burnout but less likely to produce compassion satisfaction than married clinical nurses, while other demographic and work background attributes seemed to have little to do with the nurse's compassionate feelings. (5) Younger clinical nurses showed poorer spiritual self-care; unmarried nurses showed poorer physical, interpersonal, professional, and spiritual self-care; less experienced nurses showed poorer professional and spiritual self-care; and, clinical nurses working more than 1 hour of overtime a day in a week, showed poorer physiological self-care. (6) There were significant positive correlations between compassion satisfaction and each of all five self-cares, significant negative correlations between burnout and each of all five self-cares, and a significant negative correlation between secondary traumatic stress and physiological self-care. (7) Burnout and secondary traumatic stress could significantly predict physiological self-care; compassionate satisfaction and burnout could significantly predict psychological and professional self-cares; burnout could significantly predict interpersonal self-care; and, compassionate satisfaction could significantly predict spiritual self-care. (8) The degree of spiritual self-care could significantly predict compassion satisfaction; the degrees of physical and spiritual self-cares could significantly predict ㄍburnout; the degree of physical self-care could significantly predict secondary traumatic stress. Based on the above research findings, the researcher makes recommendations for clinical nurses, medical institutions, and future research.
