羞恥被認為是形成及延續酒精使用障礙症(AUD),及使酒精使用情形惡化、提升復發風險,乃至於造成對治療的抗拒、退出治療而不欲改變之負向情緒;雖然羞恥為華人文化所重視的情感,但羞恥情緒對華人AUD者的影響尚缺少研究探索。本研究旨在探究復原階段成年男性AUD者的羞恥經驗及主體意義,以及羞恥對飲酒狀況之影響,研究目的試圖瞭解:(一)復原階段成年男性AUD者的羞恥經驗及其置身理解。(二)復原階段成年男性AUD者的羞恥經驗對飲酒情形及生活經驗之影響。(三)復原階段成年男性AUD者的婚姻家庭關係與生活世界之處境結構。研究方法採取詮釋現象學的研究立場與現象學方法論,對四位進行酒癮治療的已婚成年男性AUD者進行半結構訪談,將訪談資料登打為逐字稿後進行現象學分析。本研究有五項主要研究發現:(一)男性家庭人在婚家關係的處境結構。(二)男性家庭人的受苦經驗及形成脈絡。(三)羞恥的置身經驗及存在意涵。(四)飲酒的異變經驗及其曖昧性。(五)倫理療癒的復原歷程。研究結果顯示羞恥的置身經驗為無以容身及無從連結他人的困厄處境;羞恥對本研究AUD者產生殊異的影響,既有促進改變、關係修復行動及形塑社會-道德形象之主體形構作用/歷程的存在意涵,也可能造成迴避面對飲酒議題及關係中的退縮行為。最後,針對本研究發現提出心理實務工作及未來研究之建議。
Shame, a negative emotion, is usually considered as the antecedent and maintenance of alcohol use disorder, and the factor exacerbating the condition of people’s alcohol use as well as increasing the risk of relapse, dropout rates, resistance to treatment and unwillingness to change. Although shame plays a pivotal role in Chinese culture, the influence of ashamed experience of Chinese people with alcohol use disorder(AUD) is rarely studied. This research aims at understanding the subjective experience and meaning of shame and its influence on married adult males recovering from AUD. Semi-structed interviews are conducted to four adult males recovering from AUD. Collected data is analyzed through the lens of hermeneutic phenomenology. This research has five major findings: (1) situated structure of being a male “family person” in marital-familial relationship, (2) experience and formation of suffering of male “family person”, (3) existential meaning and situated experience of shame, (4) transformational experience and ambivalent meaning of drinking alcohol, and (5) recovery process as ethical healing. The results indicate that ashamed experience is manifested as existential predicament for people recovering from AUD, causing both positive and negative influence on one’s AUD. Last, clinical and research implications are suggested.
